A clinician documents a skilled assessment at the beginning of a shift, but things can change dramatically over the next eight, 10 or 12 hours. Some changes might include the place that care is being provided (school, home, traveling to physician appointments), tolerance of current treatments, and the client condition. The Axxess HomeCare team has created a continuous care flowsheet that makes it easy and efficient to document these frequent changes.
Easy and Efficient
The new flowsheet is divided into body systems, so that updates can be documented on a specific system or topic (i.e. education), and the other sections would not need to be addressed if there are no changes. There is also an option to quickly document that there are no changes from the previous assessment.
This option would be a notification that the body system was re-assessed, but that there were no changes noted. This style of the flowsheet enables a clinician to quickly document changes or updates, as well as notate that there were follow-up assessments, but no changes were observed. This has been done using the narrative note, but the new flowsheet is a more efficient way to document care.
This flowsheet will enable the clinicians to spend less time documenting and more time assessing and caring for patients. This will also make it easier for clinicians to coordinate care with care team members and provide education to clients and families.
