This advanced tool is designed to optimize OASIS documentation, enhancing both accuracy and efficiency.
To select which audit types you want the OASIS scrubber to run, navigate to Company Setup under the Admin tab. On the Clinical tab, select the desired audit types under OASIS Scrubber Audit Types. Click Submit Request at the bottom of the screen to save these settings.

Once an OASIS is complete, click Check for Errors to run an OASIS audit.
If no errors are detected, the signature screen will display. Click OASIS Scrubber to download a PDF copy of the OASIS audit findings.
All flags detected by the OASIS scrubber will be listed on the PDF.
The Avoidable Event flag identifies responses on an assessment that will be flagged by CMS as Potentially Avoidable Events (PAEs).
The Clinical Inconsistency flag identifies possible inconsistencies between OASIS responses on an assessment. These may or may not need to be adjusted.
The Diagnosis Coding flag identifies possible inconsistencies between a diagnosis and other OASIS items. It also identifies instances where the diagnoses may not conform to coding rules, particularly as they relate to PDGM.
The Fatal Audit flag identifies issues that would cause CMS to reject the OASIS. This flag will typically not allow the user to complete the OASIS until corrected.
The CMS Warning flag identifies warnings that do not cause rejection. These warnings will also appear on the signature page of the OASIS with a yellow triangle symbol beside them.
The Process Measure Audit flag identifies responses to process items that indicate best practices have not been followed.
The Compliance Risk Audit flag identifies instances where the pattern of OASIS responses and planned care patterns have strong potential for triggering a post-payment review.
The Outcome Potential flag identifies situations where the organization has potentially underassessed a patient’s needs, based on the seven OASIS items where improvement over the course of care is measured.
The Grouper Validity Audit flag identifies OASIS responses or omissions that block the generation of a valid HIPPS score. Such omissions may also create fatal audits. This typically also generates an error on the signature page and will prevent the OASIS from being completed until corrected.
The Readmission Risk Audit flag identifies OASIS responses that are common among patients who are especially likely to experience an early return to the hospital.
