Medi-Span Mobile

Axxess Home Health has transitioned to the Medi-Span drug database. Medi-Span improves patient care by helping users make informed decisions and provide results that are relevant to the clinical decision-making process.

To add a medication on a mobile device, users must have permission to manage medications in Axxess Home Health.

Manage Medication Profile/History/Snapshot/Pharmacies Permission

To give a user permission to manage medications, edit the user’s profile and navigate to the Permissions tab. In the Clinical section, select Manage Medication Profile/History/Snapshot/Pharmacies. Click Save to finish granting the permission to the user.

Readmit Patient Workflow

To access the medication profile for patients with no active medications but discontinued medications, navigate to the patient’s chart, a clinical task or a physician order.

On the Medication Profile screen, select Add New Medications and enter the medication name in the Search Medications Here box. Users can select the desired medication from the drop-down menu.

Users on the mobile app attempting to activate a discontinued medication from the previous database will receive a notification alerting them that the action is not available on a mobile device. Instead of accessing the website to activate the discontinued medication, users can add the discontinued medication as a new medication on their mobile device.
