The new Referral Form is more concise and allows you to process referrals quicker and carry information over to New Patient Form once an admission decision is made.
Referral Source
- Ability to identify and distinguish the certifying physician from the primary physician if the referring physician is different than the PCP and is the one signing/certifying the Plan of Care.
