The new Referral Form is more concise and allows you to process referrals quicker and carry information over to New Patient Form once an admission decision is made.

Referral Source

• Ability to identify and distinguish the certifying physician from the primary physician if the referring physician is different than the PCP and is the one signing/certifying the Plan of Care. Will also identify who is the primary physician on the POC so that subsequent orders will be sent to the PCP. [CoP -484.55, 484.60]
