Axxess Home Care now enables users to create care plans up to 30 days before each new care period begins. By creating a care plan before an upcoming care period begins, organizations can streamline documentation processes for continued compliance and seamless care continuity.

To create a care plan for an upcoming care period, create a custom care plan for the client and select the upcoming care period from the Associated Care Period drop-down menu under Visit Details.

If an upcoming care period has been created, the upcoming care period and current care period will appear in the Associated Care Period drop-down menu. If an upcoming care period has not been created, only the current care period will appear in the drop-down menu.

Create custom care plan ➜ Visit Details section ➜ Associated Care Period menu ➜ Select upcoming care period

Data entered on the care plan for an upcoming care period will flow to the associated notes beginning on the start date of the new care period. Custom visit notes will continue to display the current care plan items.
