Axxess Palliative Care now provides a Continuity of Care section in all visit notes. When completing a visit note, this new section enables users to identify which providers should receive a copy of the visit note upon completion.
Once these providers are identified and the visit note is completed, users can navigate to the new Continuity of Care screen under the Patients tab to complete the sending process in the provider’s preferred delivery method.
By optimizing provider communication and continuity of care, this enhancement streamlines operations, facilitates compliance and improves the patient experience.
When completing a visit note, navigate to the Assessment and Plan tab to document which providers/facilities should receive a copy of the completed note. In the Continuity of Care section, select the desired providers/facilities from the Provider and Facility drop-down menus.

Once the visit note has been completed and approved by QA, it will appear on the Continuity of Care screen under the Patients tab.
Navigate to the Continuity of Care screen under the Patients tab to complete the sending process.

The delivery method identified in the provider’s setup will populate in the Delivery Method column. To update the delivery method for a completed visit note, click Update Delivery Method under Actions.
To download a visit, click the download icon ( ). To print a visit, click the print icon ( ).
Once a visit has been printed and sent in the chosen delivery method, it can be marked as sent. To mark a visit as sent, click Mark as Sent under Actions. Once the visit has been marked as sent, it will appear on the Sent to Recipient tab.
