Axxess Palliative Care now enables users to document hospitalizations and emergency department encounters in each patient’s chart. Documented hospitalizations will automatically flow to the Hospitalized Patients Report and emergency department encounters will automatically flow to the new Emergency Department Encounters Report.
These enhancements streamline the documentation and tracking of hospitalizations and emergency department encounters, so organizations can monitor patient health statuses and adjust care as needed.
To document a hospitalization or emergency department encounter in a patient’s chart, users must have permission to view and edit patient charts in Axxess Palliative Care.
To give a user permission to view and edit patient charts, edit the user’s profile and navigate to the Permissions tab. In the Clinical section, select View and Edit next to the Patient Chart permission. Click Save to finish granting the permissions to the user.

To document a patient’s hospitalization or emergency department encounter, navigate to the patient’s chart and select Edit Profile. On the Health History tab, navigate to the Hospitalizations/Emergency Department Encounters section.

Select Add Hospitalization/ED Encounter to add a hospitalization or emergency department encounter.
Complete the required fields in the Add Hospitalization/Emergency Department Encounter window and click Save.
The saved hospitalization or emergency department encounter will appear in the Hospitalizations/Emergency Department Encounters table. To edit a hospitalization or emergency department encounter, select the edit icon under Actions.
The hospitalizations you document in this section of a patient’s chart will appear in the Hospitalized Patients Report. The emergency department encounters you document here will appear in the Emergency Department Encounters Report. To access these reports, users must have permission to view clinical reports in Axxess Palliative Care.
To give a user permission to view, export and delete clinical reports, edit the user’s profile and navigate to the Permissions tab. In the Reports section, select View, Export and Delete next to the Clinical Reports permission. Click Save to finish granting the permissions to the user.

To access the Hospitalized Patients Report, navigate to the Report Center under the Reports tab. In the Clinical Reports section, select Hospitalized Patients Report.
