Log in to your electronic health record (EHR) system.

Generate a CAHPS report for the sample month, and save the CAHPS report file for uploading.
If your organization does not use an EHR system, you can create a CAHPS patient information file manually. The Upload File Page in your Axxess CAHPS portal provides a downloadable File Specifications Template and guidance document for creating patient information files.

Log in to the Axxess CAHPS portal.

Select the Admin Tools tab at the top of the screen, and select Upload File from the drop-down menu.

On the Upload File page, select your location and click Choose File to select your patient file.

Click the red Upload button to complete the upload process.
