Interdisciplinary Group Center


The Interdisciplinary Group (IDG) Center in Axxess Hospice, powered by Axxess intelligence™, automates the generation of patient categories for each meeting. This enhancement ensures compliance with regulatory requirements, minimizes the risk of overlooking patients for care review and lessens the administrative burden associated with preparing and conducting effective meetings. Clinical checks for each IDG category streamline the necessary documentation for compliance and billing purposes.

IDG Center Overview


The IDG Center gives users complete control over IDG meeting processes. All information is accessible and consolidated in a central location for easy management of all IDG-related tasks with the assistance of Axxess intelligence™. Features that help users streamline IDG operations include:


  • IDG meeting scheduling automation

  • IDG summary and agenda automation

  • IDG prep report for seamlessly auditing required clinical documents prior to IDG meetings

Accessing the IDG Center


To access the IDG Center navigate to the Centers tab and select Interdisciplinary Group (IDG) Center from the drop-down menu.


Teams are listed on the left side of the screen. Users can filter by branch or search to locate a specific team.


Select a team from the menu to view details. At the top of the screen, the team name and team members are displayed along with the number of Recertifications and Unassigned Patients. Select Edit Team to update team information.


Below the banner, users can select from the following tabs to view: Upcoming Agenda, Meetings, Team Members, Groups and Patients.


Upcoming Agenda


The Upcoming Agenda tab displays information for the team’s next IDG meeting. Using Axxess intelligence™ patients are categorized into the following sections based on their activity in the software:


  • Death

  • Discharge

  • Admission

  • Recertification

  • Change in Level of Care

  • Active



Meetings


The Meetings tab displays all meetings that have occurred and are scheduled for the team. In the Actions column, select View Meeting to view the IDG Meeting Details screen, where the agenda for the meeting can be accessed and printed.


Team Members


The Team Members tab displays a list of users who have been added to the team. Message User under the Actions column enables users to message any team member directly from the screen.


Select Edit Team at the top of the screen to add or remove members of the team, as well as update team details.


Groups


The Groups tab displays the group name and hospice physician of each group within the team. Click Edit Team and select the Groups tab to update the team’s groups.


Patients


The Patients tab displays the following information for each patient within the team:


  • Patient Name

  • MRN

  • Recertification Date

  • Group


Patients are listed in descending order with the most urgent recertification date at the top of the list.


A badge next to the patient’s name alerts users to upcoming and past due recertifications dates:


  • Upcoming: 30 to 15 days before recertification due date

  • At Risk: 14 to one day before recertification due date

  • Past Due: Recertification due date has passed


To edit patients on the list, select Edit Team and navigate to the Patients section, select Update to reassign a patient’s group or team. Users can also change an individual patient’s care team from the Patient Profile.
