Axxess Hospice now provides three new reports that organizations can use to gain insight into their business. The new reports, Days Without a Skilled Nursing Visit, Skilled Nursing Care Visits and Visits Near Death, help organizations track items that are part of the Hospice Care Index as required in the CMS Final Rule for FY 2022.
To access these reports, users must have permission to view statistical/clinical reports in Axxess Hospice.
To give a user permission to view statistical/clinical reports, edit the user’s profile and navigate to the Permissions tab. In the Reports section, select View next to the Statistical Reports and/or Clinical Reports permission. Click Save to finish granting the permissions to the user.

To access the new reports, navigate to the Report Center under the Reports tab.

The Days Without a Skilled Nursing Visit report tracks the number of eight or more days between skilled nursing visits to patients which is one of the components of the Hospice Care Index measure.
Once a user has navigated to the Statistical Reports section and selected the Days Without a Skilled Nursing Visit report, the report will open in a new window with the following parameter options:

The Skilled Nursing Care Visits report displays the total number of minutes of skilled nursing visits for a given time frame and visits that occurred on weekends. This report helps track two of the components for the Hospice Care Index measure.
Once a user has navigated to the Clinical Reports section and selected the Skilled Nursing Care Visits report, the report will open in a new window with the following parameter options:

Once generated, users will see the Total Nursing Minutes calculated and a breakdown of the minutes on weekend and weekday visits.
The Visits Near Death report displays visits by the LVN/LPN, RN or MSW during the last three days of the patient’s life. Once a user has navigated to the Statistical Reports section and selected Visits Near Death, the report will open in a new window with the following parameter options:
