The following functionality is coming soon to Axxess Hospice. Axxess will alert you when this feature is available. Check your messages in Axxess Hospice for important alerts from Axxess.
Axxess Hospice now prompts users to update the patient’s service location when completing a discharge, readmission, recertification, or level of care change through a physician order. New fields also appear in the Service Locations section of each patient’s chart, so users have a more comprehensive view of patients’ service locations. These enhancements streamline service location documentation and tracking.
To edit a patient’s service location, users must have permission to view and edit patient charts in Axxess Hospice.
To give a user permission to view and edit patient charts, edit the user’s profile and navigate to the Permissions tab. In the Clinical section, select View and Edit next to the Patient Charts permission. Click Save to finish granting the permissions to the user.

To add a new service location to a patient’s chart, navigate to the desired patient’s chart and click Edit Profile. In the Service Locations section, click Add Service Location.

Next to the Service Location drop-down menu, the following new fields will appear: Facility Name, Start Date, Start Time, End Date and End Time.
The Start Date and Start Time fields are required. When Home – Q5001 is selected as the service location, the Facility Name field will display Not Applicable. All changes made in the Service Locations section will appear in the Activity Log under the Admin tab.
When Complete is selected on a physician order, the system will now ask the user if they want to update the patient’s service location. Select Yes to update the patient’s service location in the Service Locations section of the patient’s chart.
The same pop-up will appear when completing a Discharge Visit, LPN/LVN Discharge Visit, or Unattended Discharge Visit. When a pending patient is updated to active and the RN Initial/Comprehensive Assessment and RN Comprehensive Assessment are signed and dated, the same pop-up message will appear.
The same pop-up will appear when completing an RN Recertification Visit.
