Updates to Election Addendum/Non-Covered Items


Axxess Hospice now allows agencies to quickly generate a Patient Notice of Hospice Non-Covered Items, Services and Drugs addendum, as required by the Centers for Medicare & Medicaid Services (CMS).


Permission to edit Visits is required to document notification of the patient’s right to receive the election statement addendum during patient visits.


To add or update this permission, navigate to the User Profile by selecting People Center from the People menu and clicking Edit for the desired user. Navigate to the Permissions tab in the user profile and choose View/Add/Edit/Delete permissions in the Visits row of the Clinical permissions.

Permission to edit the Patient Chart and Medications are required to document relatedness of items, services and medications.


To add or update this permission, navigate to the User Profile by selecting People Center from the People menu and clicking Edit for the desired user. Navigate to the Permissions tab in the user profile and choose View in the Medication Profile row of the Clinical permissions and View/Edit permissions in the Patient Chart row of the Clinical permissions.

As part of the regulatory changes finalized in the CMS Final Rule for FY 2022, hospice organizations are required, as of October 1, 2022, to update the addendum to include a furnished date and reason why the patient/representative refused to sign if applicable.


This addendum, the Patient Notification of Hospice Non-Covered Items, Services, and Drugs, will provide the requester with an accounting of items, services, and drugs which have been determined by the hospice to be unrelated to the patient’s terminal diagnosis and related conditions and therefore not covered by the hospice.


Axxess Hospice allows agencies to document their notification of patients and caregivers of this right, document the request for an addendum, document relatedness and generate an addendum to deliver to the patient or caregiver.


Generating and Tracking an Addendum


A Patient Notification of Hospice Non-Covered Items, Services, and Drugs addendum form can be generated at any time from the patients chart in the new Non-Covered Items page. This page can be accessed from the Quick Links menu in the patient chart or by selecting Non-Covered Items from the Patients menu. 


The Non-Covered Items page allow users to generate and manage the addenda for a patient. To add a new addendum for a patient, select Generate Addendum and enter the Requested Date, Requested By, the Name of the requester and the Furnished Date. If the patient is requesting the addendum, their name will automatically populate in the name field. Once entered, click Generate Addendum to create, view, and update the addendum.


Once created the Patient Notification of Hospice Non-Covered Items, Services, and Drugs form will be pre-populated with the following information:

Select Cancel to remove any changes made to the form, click Save to save any changes, and click Complete to mark the form as ready to send to the patient/representative.


All generated forms for a patient will be visible on the Non-Covered Items page. Once a form has been completed, it will be in To Be Sent status. Select the print or download icon to print or save a copy of the form. Click the edit icon to make changes to the form. Click the delete icon to remove the form.


When the form is printed or downloaded, a signature and date line for the patient/representative and a witness signature and date line will appear. This is so when the requester reviews the form, a signature and date can be collected. The furnished date field will appear as well as a place to document if the patient/representative refuses to sign.


Once the form has been printed and delivered to the requester, select Mark as Sent from the Actions column on the Non-Covered Items page. The status of the document will then update to Furnished.


Once the form has been signed or refused to sign by the requester, select Mark as Signed or Refused to Sign to upload the document. Then enter the date signed or refused and select Save.


Once the form has been uploaded and marked as Signed or Refused to Sign, the Signature Status column will display a green check mark if signed or red x if refused to sign as well as the date.


If the Signature Status needs to be updated, choose the … under the Actions column to update the status. Once the form has been uploaded, select the download or print icon to view the completed form.
