The OASIS-C2 layout has been updated. The physical assessment and plan of care sections have additional columns added, which enable clinicians to see more information in one view, thereby improving ease of use with scrolling on the pages.
The OASIS C2 features additional tooltips and instructional text to assist clinicians with complete and accurate documentation.
A link has been added within the Hospitalization Risk Assessment section for ease of use. As per Medicare regulations, interventions must be included in the Plan of Care to mitigate the risk and prevent acute care hospitalization and emergency department visits.
Once interventions are selected, the Risk for Hospitalization indicator present in the Skilled Nurse note will be red.
We have added a new High Risk for Infection Assessment in the OASIS-C2 to identify those patients who are at risk. This information will flow to the Infection Control and Surveillance Report for QAPI. In addition, the clinician can now select a problem statement and interventions to mitigate prevention for those patients identified as high-risk.
