The Plan of Care Profile button is located under the Interventions section in the Skilled Nurse Note.
From this view, the clinician can resolve goals and discontinue interventions at the point of care to update the Plan of Care.
When "No" is selected for Advanced Directives in POC
If "No" is selected in the Advanced Directives section of the completed OASIS-C2, the Plan of Care PDF Advanced Directives section reads, "This patient does not have an advanced care plan or a surrogate decision maker and is not able to provide legal documentation for the home health medical record."
